When making an adult social care enquiry with Shropshire Council, if it is not possible to deal with your enquiry over the phone, you will be offered a date, time and place to see a skilled adviser in person to discuss your issues or queries.
The person you speak to will establish whether you need to have support during the discussion and an assessment maybe carried out during the session. You will have the opportunity to nominate a suitable person to help you. If you are unable to nominate a suitable person, the Council can assist you in finding an Independent Advocate to help provide you with the necessary support.
The adviser may need to take some notes and it will be agreed by you, what information is to be kept on record. This information will be put into a form called a Personal Profile.
The Personal Profile form asks a number of questions such as:
- What is really important to you in relation to having a good life, staying independent and living in the community
- What is working currently for you and what isn’t working and what needs to change.
Finally, a short action plan with a list of who is going to do what will be made (some or all of these actions may need to be done by you).
